• It is the parents' responsibility to inform the school nurse and teacher of any health conditions that a student has at the beginning of each school year. It is also the responsibility of the parent to make the nurse and teacher aware if any changes in health occur during the school year.  When notified, we will develop a plan of care for your student and assist you in getting all necessary medical forms filled out. After the plan of care is completed, I will make sure teachers and staff are informed and trained on how to use any emergency medications if needed. For any documented food allergies, a diet order form must be completed. The diet order form will be turned into the cafeteria so that the student account will be flagged and they will not be allowed to purchase items that contain the food(s) they are allergic to.
